Short-Term COVID-19 Accommodation Online Request Form
This form is intended for graduate students who have in-person responsibilities that they are unable to meet due to reasons related to COVID-19 and who have been unable to find a solution with their adviser and/or director of graduate studies.
This form can facilitate the review of any medical, personal, or travel issues that are barriers to physically returning to campus when in-person responsibilities resume.
If you have already identified a solution with your adviser and/or director of graduate studies, please do not submit this form.
To assist students experiencing temporary COVID-19 related issues who will be able to return to in-person responsibilities at the end of the approved accommodation period, a short-term COVID -19 Accommodation may be available. Short-term COVID-19 accommodations cannot exceed twelve (12) weeks. Graduate students who may have longer term needs that require adjustments in their topic, research project, or advising should use the approved accommodation period to work with their department on longer term academic plans that will allow them to make academic progress.
Completed Short-Term COVID-19 Accommodation Request Forms are automatically submitted to the Office of the Dean of the Graduate School. Any accompanying documentation should be submitted to the appropriate offices listed on the form.
